The Health Care Center Annex is located just two miles from the main center.
Its primary focus is provision of vocational rehabilitation services along with psycho-social services to persons from the IMD, ICF-MR, Farnam Community Living Center, Blair Apartments and to persons living in the community or in the school system who have mental illness or a developmental disability.
Staff and community education is also provided in this setting.
In addition, the Annex houses the Health Care Center's:
- Human Resources Department
- Management Information Systems Department
- Business Office for payroll and accounts receivable
- West Central Wisconsin Behavioral Health
- United Volunteer Caregivers organization
