Blair Apartments
Blair Apartments provide a supported apartment environment for adults with chronic mental illness.
The apartments are located in the pleasant community of Blair, Wisconsin. They are within walking distance of a park, pharmacy, bank, grocery store, community center, library, churches and limited shopping.

Objectives
- Individual empowerment
- Acquisition and enhancement of independent living skills
- Maintenance of mental health stability and behavioral wellness
- Integration into community living, working, learning and leisure activities
- Eventual return to independence in community or county of residency
Services included in base rate:
- Medication management, monitor and education
- Crisis intervention and emergency response services through staff on call 24 hours per day
- Vocational program participation, including training and job placement
- Local transportation
- Planned recreational outings
- Individual social and recreational skill training
- Problem-solving and decision-making education and support/supervision related to personal hygiene, ADL's, shopping, safety, household upkeep, transportation utilization and money management
Other Services
- Provision of psychiatric follow-up
- Coordination of services as needed
- Medical monitor and follow-up through the Blair Gundersen Clinic
- Assistance in procuring food stamps
- Assistance with transportation

- Registered Nurse
- Social Worker/Case Manager
- Supportive staff as required
- ADL/Vocational Day Program TCHCC staff
Apartment Offerings
- Four apartments with private entrances
- Two tenants per apartment possible
- Private bedrooms
- Furnished with basic furniture and washer/dryer
- Equipped with dishes, cookware, utensils, small appliances, linen, and household cleaning untensils and supplies
- *Electricity, heat and refuse collection
- *Note: Telephone and food are not included in basic monthly cost.
