Services included in base rate:

• Medication management, monitor and education
• Crisis intervention and emergency response services through staff on call 24 hours per day
• Vocational program participation, including training and job placement
• Local transportation
• Planned recreational outings
• Individual social and recreational skill training
• Problem-solving and decision-making education and support/supervision related to personal hygiene, ADL's, shopping, safety, household upkeep, transportation utilization and money management

Other Services

• Provision of psychiatric follow-up
• Coordination of services as needed
• Medical monitor and follow-up through the Blair Gundersen Clinic
• Assistance in procuring food stamps
• Assistance with transportation

• Registered Nurse
• Social Worker/Case Manager
• Supportive staff as required
• ADL/Vocational Day Program TCHCC staff

Apartment Offerings

• Four apartments with private entrances
• Two tenants per apartment possible
• Private bedrooms
• Furnished with basic furniture and washer/dryer
• Equipped with dishes, cookware, utensils, small appliances, linen, and household cleaning untensils and supplies
• *Electricity, heat and refuse collection
