Community Living Overview


The Trempealeau County Health Care Center Community Programs offer a diverse selection of homes to meet individual needs and enhance their quality of life. Our many residences are designed to ensure clients truly feel at home. The Trempealeau County Health Care Center system’s philosophy and mission promotes each client directing their care.


We proudly have ongoing relationships with a majority of our Wisconsin counties. Fostering those professional relationships is tantamount to our success. Living options are in rural and city locations. We currently offer ten community placements that are licensed or certified, and a semi-supervised apartment setting.

Our system offers a Crisis Bed option for those clients that are facing challenges that can be briefly managed in a safe setting. Located in our in-patient unit, the stay is designed to last for three days. The psychiatrist assesses the client and determines readiness for a return home. Utilization of this option eliminates a need to travel to a larger acute psychiatric facility and less upset to one’s life.
