Apple Way Adult Family Home

Apple Way is a four-person, licensed Adult Family Home, located on the outskirts of Independence, WI. This home accepts persons who are physically handicapped, have a developmental disability, mental illness, or are of advanced age. Apple Way was constructed in 2005 and has an inviting open concept floor plan, with wheelchair accessibility. Each person has a private bedroom.

• Medication management and monitor
• Personal care services
• Health monitoring services and appointment coordination/follow-up
• Recreation and leisure time services
• In-county transportation
• Vocational services
• Information and referral

Staffing

Staffing includes a 24-hour awake staff that includes C.N.A.’s and Personal Care Workers.  A social worker provides case management and there is access to an R.N., who is available for consultation and monitor to residents of the home.  A psychiatrist is contracted to provide consultation and follow-up.
