Clover Way RCAC

Clover Way RCAC has been certified by the State of Wisconsin, serving a focus population of tenants with chronic mental illness. It is located on the outskirts of the city of Independence, WI.  There are seven efficiency apartments that are fully furnished, right down to dishes, towels, linens, etc. Tenants will only need their clothing and personal items.  These apartments are private, with each apartment occupied by one person. There is a common kitchen/dining room, a living room, and an office.

Monthly Rate

The total monthly rate is the combined charge from rental, basic services, and optional service charges. Up to 24 hours/week of nursing services are allowable under the base rate. A security deposit will be waived if the tenant agrees to sign a statement assuring responsibility for damages.

Services

Services within the base rate include the following:

• Housekeeping including cleaning the apartment weekly and thorough cleaning
  (walls, carpets, windows) semiannually.
• Apartment maintenance
• Continental breakfast, noon and evening meals
• Laundry services—linens, and towels
• Utilities except long distance calls (local calls provided), cable TV and internet
• 24 hour per day emergency response
• Assistance with dressing, grooming, bathing, toileting, transferring, and ambulating
• Care plan management
• Medication management
• Vocational programming
• Activities/Recreation
• Transportation (does not include medical transport)
• Tenants may choose to buy and prepare their own food within their apartment.
