Farnam Community Living Center is a licensed 18-bed
Community Based Residential Facility. We are located in
the heart of Whitehall, WI. We pride ourselves in providing
a comfortable and caring environment at a reasonable

Criteria for Admission
- At least 18 years of age
- Must have mental illness
- Able to perform basic independent living and social skills
- Willingness to cooperate with a treatment plan
- Able to self-supervise for periods of time each day
- Basic ability to structure leisure time
- Ability to negotiate stairs
