Adult Family Homes

Our licensed adult family homes are located in quiet neighborhoods between Eau Claire and La Crosse.

Our homes are recently built and provide care for adults with developmental disability, mental illness and/or physical disability. Each home provides a spacious congregate living area as well as private bedrooms for four individuals.

Objectives

• Individual empowerment based on needs with support of quality services
• Integration into community living, working, learning and leisure activities
• Maintenance and enhancement of individual living skills

Services Included in Base Rate:

• Personal care assistance
• Medication and health monitoring
• Information and referral services
• Recreation and leisure time activities
• Employment through a vocational rehab program located at the Annex
• Meals and housekeeping
• Treatment team may include psychologist, psychiatrist, social worker, registered nurse, and special program staff
