Supportive Apartments
Supportive Apartments, located across the road from the Trempealeau County Health Care Center, provides residency in an adult family home setting. The houses (Elmwood, Maplewood, Oakwood and Pinewood) each have two tenants living in a supervised environment. The accommodations are fully furnished and provide a private bedroom for each resident.
Objectives
- Individual empowerment based on needs with support of quality services
- Integration into community living, working, learning and leisure activities
- Maintenance and enhancement of individual living skills
- Enhancement and acquisition of skills necessary for independent living
Criteria
- Capability to perform basic independent living and social skills
- At least 18 years of age
- Willingness to cooperate with a treatment plan
- Capability to self-supervise for blocks of time each day
- Basic ability to structure leisure time
- Ability to negotiate stairs
Supportive Services
- Medication management (monitoring and education)
- Crisis intervention and emergency response services
- Vocational Program participation including vocational training and job placement
- Psychiatric follow-up
- Local transportation
- Social and recreational activities
- Information and referral support
- Problem-solving and decision-making education
- Meals and snacks
Staffing
- 24-hour awake staff
- Personal Care Workers or Certified Nurse Aides
- RN assessment and supervision
- Contracted psychiatrist for follow-up and consultation
