Trempealeau County Health Care Center

Trempealeau County Health Care Center is a county owned facility with roots beginning in 1898. Construction projects and razing of the 1898 building have resulted in a modern building located in a rural setting.

 

Using a recovery and person-directed care approach the Trempealeau County Health Care Center focuses on assisting persons prepare for community living through promoting independence and self-determination.  There are three licensed long term care units at the Trempealeau County Health Care Center's Main Facility.

Operations at the Main Center:

Institute for Mental Disease-Nursing Facility (IMD-NF)

  • One of two licensed IMD's in the State of Wisconsin
  • Provides services to persons throughout the State of Wisconsin over the age of 18 who have serious, persistent mental illness
  • Treatment focus is recovery based and person-directed with an emphasis on returning to community living

Nursing Facility

  • Focus is on skilled nursing care, treatment and services to persons with mental illness, dementia, organic brain syndrome and difficult behaviors or complex medical needs

Intermediate Care Facility for the Mentally
Retarded/Facility for the Developmentally
Disabled (ICF-MR/FDD)

  • Provides services to persons throughout the State of Wisconsin who are over the age of 18 and have a primary diagnosis of developmental disability and who require extensive specialized care
  • Focus is on assisting persons to function with as much self determination and independence as possible and preparing for return to community living
